provides outcomes data collection and report services across the
rehabilitation continuum. We offer standardized measurement
instruments, which are patient focused, easy to use, statistically
valid, clinically appropriate, cost effective and efficient. Our
reports are risk adjusted which offers FOTO providers a "fair
and level" playing field for national benchmarking and data

The
data collection mechanism is easy to implement. We understand the
documentation requirements in the clinic, and we strive to minimize
the respondent burden of collecting this valuable information. The
Intake involves obtaining functional and demographic information,
primarily from the patient and with minimal staff participation. The
same information is collected from the patient at discharge. The
information required from staff at discharge are variables normally
collected in rehabilitation, such as cost, visits, etc.
FOTO
provides a risk adjusted national comparison for patients,
clinicians, and programs. These reports are available in an
electronic or paper format.
Comparative
Outcomes Reports:
FOTO provides outcomes profile reports, which are risk adjusted,
easy to interpret comparisons to the national rehabilitation patient
population. If your scores are below the mean of the database, you
have a practice management tool. If your scores are above the mean
of the database, you have a marketing tool.
Custom
Reports:
FOTO offers participants the option of adding custom reports to
their standard outcomes profile. Some common sorts for these custom
reports are by clinician, referral source, payment source, or ICD9
code. Regional comparisons are also available.
Patient
Specific Reports:
Patient Specific Reports are also available at each point during the
patient's episode of care. These reports print immediately upon the
completion of the appropriate survey and can be used to supplement
or replace other forms of documentation. At intake, a narrative
patient history is produced, as well as a report that predicts the
result of treatment for this patient. During treatment, a trend
report is available at any time that graphically depicts the
progress of the patient during treatment. At the end of treatment,
the result of care is recapped and compared to a risk-adjusted group
of similar patients from across the country. There is also a
comparison of the efficiency of treatment (visits and duration) and
a detailed recap of patient satisfaction.
Patient
Evaluation Software:
FOTO has developed the most advanced patient evaluation software
available, Patient Inquiry®. Patient Inquiry®
is a “must have tool” for the clinician who wants to maintain an
evidence-based practice. The experts report that Pain, Function/QOL,
Return to Work, Patient Global Assessment and Patient Satisfaction,
as reported by the patient, are clinically important in the
evaluation of a patient’s treatment outcome.
Patient Inquiry® contains all of these parameters
in a flexible and efficient survey software.
