FOTO Outcomes Surveys can now be completed by the patient outside of the clinic (if internet access is available).
Think about the convenience of being able to have all the great functional information and predictions available to you before the patient even comes in for the first visit – and not spending time in the clinic gathering that data. In addition to the FOTO assessment, you could add pain questions, a patient history, or many other types of assessments (including ones for depression, somatization, and fear avoidance of work) to the standard FOTO questionnaire. Follow up patient surveys can also be completed online.

In the subject line put: Try online patient survey. (I will send an e-mail with a link so that you can take the intake survey just like the patient would receive.)
After you complete the survey, I will e-mail you the report that would normally be printed immediately in the clinic.
